Integrated approach of PHC
Definition:
‘Integrated approach of PHC’ means functional integration of health and family planning services under the package of PHC, which was launched under the ‘Intensified PHC Program’ in 1988.
The first evaluation of the national HFA strategy in 1986 revealed that due to various constraints such as inadequacy in the managerial process, lack of adequate resources, bias towards curative medicine, lack of coordination and community involvement, the concept and principle of PHC could not be translated into action. As such, in 1988 the government initiated the program known as “Intensified PHC Program”. The Intensified PHC programme has demonstrated success in developing useful working mechanism at Upazila level and below by adopting –
- Functional integration of health and family planning services under the package of PHC b. Community mobilization through the involvement of Village Health Volunteers (VHB) and TBAS
- Inter-sectoral action through “action committees” at different levels; and
- Strengthening of project management at union, upazila and district levels.
Role of community participation in PHC
Community participation has moved a step further from passive recipients to active participation through the Village Health Volunteer (VHV) Scheme. The Volunteers are selected by their communities on the basis of representation (1 Village Health Volunteer represent 30 household clusters).
Their role includes education of community members on the basic causes, prevention and control of diseases and referrals. VHVS identify cases within their clusters for treatment and personally accompanying them to the Village Health Care Posts or Union Health and Family Welfare Centers. During outreach clinic sessions they assist Health Assistants and Family Welfare Assistants (FWAS) at Village Health Care Posts with growth monitoring, social mobilization, distribution of Vitamin A and ORS. Screening and referral roles of VHVS in Bangladesh are unique and have added impetus to the whole concept of volunteers in PHC.
Community Health Problems in Bangladesh:
- Diarrhoea and intestinal parasitic diseases
- Respiratory diseases (ARI, pneumonia) 3. Infectious diseases of childhood (especially measles and polio)
- Other infectious diseases (especially HIV/AIDS, tuberculosis and malaria)umm 5. Malnutrition: PEM, iron deficiency anaemia, night blindness and ricketsents Jad
- Complications of pregnancy and childbirth
